The Mehlville School District will register kindergarten students for the 2012/2013 school year from January 23-January 27, 2012.  The registration will occur at the individual schools. Children born before August 1, 2007 may register for kindergarten (no exceptions).

Parents and/or legal guardians will need to call the school your child will be attending to register.  Appointment times will be available during the regular school hours and also for the evening of Thursday, January 26, from 4:00 to 6:00 p.m.

IMPORTANT:

Full-day kindergarten spots WILL NOT be assigned on a first-come/first-serve basis. A random drawing will be done to determine final placements and every attempt will be made to keep siblings together. The full-day kindergarten price for the 2012/2013 school year is $2,275.00. We require a deposit of $275.00 at the time of registration. If paying by check, please make it out to “Mehlville School District”. Eight (8) payments of $250.00 each will be expected the first of each month.

Please bring the following documents at the time of registration:

1. Child’s birth certificate issued by the Bureau of Vital Statistics
2. Medically documented immunization records
3. Proof of parent/guardian’s residency. Must have ONE item from EACH column:

Note: If parent/guardian is living with a Mehlville resident, a notarized Building Affidavit of Proof of Residency must be filled out by the parent/guardian AND the Mehlville resident. The affidavit is located at the school and must be notarized by the school secretary. Proof of residency is required for the Mehlville resident. If parent/guardian is not living with the child and the child will be living with a Mehlville School District, a meeting with Dr. Scott Hayes, Executive Director of Special Services is required to discuss residency. Please call 467-5229 to arrange a time when you can meet with him. Please bring proof of residency to your meeting.

4. Deposit of $275.00 made out to Mehlville School District, if registering for full-day kindergarten.
5. Custody papers (court appointed documents, divorce decree, DFS placement papers, foster care) which states custody rights.
6. IEP and evaluation, if the student is receiving Special Education Services.

In addition, parents and/or legal guardians will be asked to provide the name, address and phone numbers of the child’s doctor, along with the phone numbers of parent and/or legal guardian’s employers and two emergency contact names and phone numbers. Until all of the above material is received, your child cannot be enrolled in the Mehlville School District.

Registration packets can be picked up at all Mehlville Elementary Schools, or the Enrollment form and Parent Link Registration form can be downloaded from our website, at www.mehlvilleschooldistrict.com. Please come to your appointment with all necessary paperwork filled out.

All kindergarten students entering Mehlville School District must provide a record of physical examination to their attendance school prior to August 1, 2012.
